Order a Printed Directory

Only State agency telecommunications representatives officially designated as such with the ITS Division of Telecommunications may order ITS directories using the CS 70 form which is used to initiate a journal voucher. These forms may be obtained from the Division of Telecommunications at 518-474-8974 or at the ITS website JV Request for Phone Directory Chargeback Form.

Members of the public may purchase a directory(s) by visiting the Receptionist in Room 120, Concourse Level, Governor Nelson A. Rockefeller Empire State Plaza in Albany. Walk-in customers may purchase a directory for $3.50 each by check or money order made payable to the "Office of Information Technology Services." No cash transactions will be accepted. We can make arrangements for large orders to be picked up from our dock level. Do not add sales tax.

Mail-in requests incur an additional cost for postage, handling and materials. We do not accept purchase orders. Send only check or money order for $8.25 for the first directory, and $4.50 for each additional directory in the same order payable to the "Office of Information Technology Services" to:
